Help with putting my mum's care plan in place

My mum has had Parkinson's for a few years now, a few weeks ago I came up to visit with my daughter and I realized her health has declined rapidly and she is not getting the proper care she needs. I couldn't live with this so the next day I moved back up here to get her into a position she is getting the care and support she needs. I was quite naive in my thinking as this is taking a lot more work than anticipated and she is also a lot worse than I thought. I've tried to get a few jobs but have to leave after a few days due to the amount of appointments, calls and visits due her not receiving proper care before this. I moved back into her house in a mouldy cupboard and sleeping on a deck chair which is taking its toll massively on my mental health. All I'm looking for is some help for the immediate future so I can focus on getting her the right care plan put in place and the aids she needs (she's not had a bath in god knows how long). She regularly sees things and believes things that aren't true, she can barely move and the council have been little to no help which is heart breaking. Any help at all is appreciated.
