The need to change how healthcare is delivered based on the challenges faced within the local communities is the yardstick for this project.

Every individual has the right to live they way they want and a say in the care they get according to the Safeguarding Act.

Itoju-Note will provide an opportunity to  vulnerable people and parents of children under the age of 18  in keeping updated on the card plan, medical report, prescription reminders through SMS and Mobile App whilst providing primary healthcare providers an avenue to access medical report from anywhere making it easier for and continuing care and ease of diagnosis.

The associated risks include

Technical knowledge and usage

To solve the identified risks, regular training will be provided to existing and new members of staff on how to support the minority population.

And lastly, regular bi-monthly audits will be carried out to ensure data is been used to support those in care.

The benefits outweighs the associated risks, not only will child and maternal mortality rate be reduced, it will also support an ongoing service of support the vulnerable amongst us.
