End of life project Trafford Carers Centre
Trafford carers centre are starting a new project, supporting carers who are caring for someone in the last year of their live. We are hoping to provide a designated caseworker who will be supporting the carers and their families. We will be able to offer:
a Carers assessment
Provide information on relevant local services that would benefit the carer.
Help the carer understand the dying process
Reduce isolation and fear
Ensure that carers have direct access to advice on financial planning and welfare benefits
The support from the project will continue after the death of their loved one, providing bereavement support.
Caring for someone at home who is terminally ill puts a huge amount of stress on the carer, if carers are unsupported that stress can frequently become too much, leading to carer breakdown. This can lead to emergency admission to hospital for the “cared for” and the patient and carer do not achieve their preferred place of death.
Anyone can refer a carer to our services the project with support carers caring for someone in the last year of their lives, including carers of children and young carers
